Physiotherapy Assessment

The physiotherapists are now offering assessment appointments to any patients with new problems. You do not need to see the GP to make an appointment.

Please ask for a form at reception or you can download the form.

Physiotherapy Self Referral Form

Once you complete the form you can hand it to reception or email it to or post to: AHP Central Appointments. 1st Floor Education Building, Bishop Auckland General Hospital, Cockton Hill Road, Bishop Auckland, Co Durham, DL14 6AD

You will be contacted directly by the Physiotherapist. If you need more information pick up a leaflet from reception.
